Inequities in the Aging Process and Ways to Address
Rachel Zimmer, DNP, NP
Dr. Rachel Zimmer is a practicing adult-gerontological nurse practitioner specializing in care for underserved populations and older adults. She is founder and director of the Atrium Wake Forest Baptist mobile health program, serving people who do not have health insurance. She also founded a food prescription program which provides locally grown food for older adults with food insecurity. Much of her work focuses on community engagement to develop innovative solutions to address the health needs of people in underserved communities. Her scholarly work centers around health equity, the care of older adults, and addressing food insecurity.For this program, Dr. Zimmer will discuss the impacts of social drivers of health on the process of aging and will provide insight into issues facing older adults in our community. She will also discuss some programs she and others at Atrium Wake Forest have developed to help address social drivers of health that affect patients and communities.
