National Healthcare Decisions Day (NHDD) is April 16^th. NHDD is dedicated to encouraging all adults to plan for the future and to make their healthcare wishes known to family, friends and healthcare providers. In honor of this day, we are encouraging everyone 18 years old and older to take charge of their healthcare decisions. Choices and Champions at Novant Health is a resource dedicated to creating awareness, providing education, and assisting individuals with their advance directives through Advance Care Planning (ACP).  At our core, Choices and Champions is about knowing and honoring patient’s wishes by first knowing what matters most to them. We are available to start goals of care conversations and recommend the appropriate advance directive or medical order based on the individual’s values and goals to ensure they receive the care that is right for them.
